Contact allergen sensitivity in children with contact dermatitis

This retrospective series patch tested 80 Turkish children aged 1 to 17 years with contact dermatitis, using a ready-to-use 36-allergen patch-test panel, between March 2017 and February 2018.

This retrospective series patch tested 80 Turkish children aged 1 to 17 years with contact dermatitis, using a ready-to-use 36-allergen patch-test panel, between March 2017 and February 2018. A positive reaction to at least one allergen was recorded in 46 children (57.5%). Table 2 prints nickel sulfate at 15% (12 children) and potassium dichromate at 3.8% (3 children). The abstract and discussion print the nickel figure as 14.8% for the same count of 12. These are shares of children with a positive patch test. They are not concentrations of nickel or chromium in a product. No heavy-metal concentration is reported. Cobalt chloride was included in the test panel, but no cobalt positive rate is printed.

Methods (brief)

File review of children with contact dermatitis who received a TRUE test. Tapes were removed at 48 hours and the site was read again at 72 or 96 hours by the study allergists using the American Academy of Dermatology reaction scale. Patients with concomitant atopic dermatitis were excluded. Nickel-positive and nickel-negative children were compared on age, sex, symptom duration, pruritus score, IgE, and eosinophils.

Evidence fitness

Patch-test positive rates for nickel sulfate and potassium dichromate in this Turkish pediatric contact-dermatitis cohort.

They do not support Nickel, chromium, or cobalt concentrations in wet wipes, moisturizers, diaper rash creams, or any other product; A cobalt sensitization rate. Cobalt chloride is named on the TRUE test panel, but Table 2 does not print a cobalt positive count or percent; A measured nickel release from products. The 0.5 micrograms per square centimeter per week figure is a cited European Union recommendation, not a result of this study; Sex-specific nickel sensitization rates. Table 4 prints the sex composition of the nickel-positive group, not the percent of girls or boys who were positive.

Limitations

Single center, one year, and 80 children. Atopic dermatitis was an exclusion, so the cohort is not children with atopic dermatitis. The authors state that the wide spread of positive allergens prevented stratification by lesion site. Table 2 appears to list detected allergens only, so an unlisted allergen cannot be read as a printed zero.
