Plant-based diet in hyperkalemic chronic kidney disease patients receiving sodium zirconium cyclosilicate: a feasibility clinical trial

This 6-week Swedish feasibility trial gave 26 patients with stage 4 to 5 chronic kidney disease and high blood potassium the potassium binder sodium zirconium cyclosilicate while they moved from a low-potassium diet to a high-potassium plant-based diet. Plasma potassium stayed mostly in the normal range. The trial did not measure zirconium in blood, urine or any other matrix.

Key numbers

  • Dose: 30 g/d of sodium zirconium cyclosilicate for the first 48 to 72 h, then 5 to 10 g/d titrated to plasma potassium; the most frequently prescribed dose was 10 g/d (Methods, p. 720; Results, p. 722).
  • Mean plasma potassium fell from 5.5 to 4.4 mEq/L within 48 to 72 h and then ran 4.7 to 5.0 mEq/L; 58% of patients stayed between 3.5 and 5.0 mEq/L (Abstract).
  • Ten mild, transient gastrointestinal events were recorded; the physician rated 6 as probably or possibly related to the drug (Results, p. 722).
  • This paper reports no numerical findings relevant to zirconium exposure.

Methods (brief)

Single-arm 6-week trial in 26 patients (Karolinska, Sweden): 3 weeks low-protein low-potassium diet plus the drug, then 3 weeks of a plant-based diet delivered as weekly food baskets while continuing the drug. Weekly plasma potassium, 24-hour urine, food records, nutritional status, stool scale and quality of life.

Evidence fitness

Context only: documents an oral pharmaceutical use of a zirconium compound and short-term gastrointestinal tolerability in a small trial. It does not measure zirconium absorption or body burden, so it does not support zirconium exposure or toxicity claims.

Limitations

Small single-arm feasibility trial without a control group. The study was funded by AstraZeneca, which was involved in the study design; several authors report honoraria from AstraZeneca and other companies, and the lead unit is funded by a Baxter Healthcare grant.
